Were Mai & Zuko together before his banishment?

In a sense yes. But I think they really came together after the fall of Ba Sing Se to the Fire Nation. And that too was part of Azula's manipulation and master plan. She used Mai to spy, study and and extract information on Zuko. When he's most vulnerable, when he's most open. That was her original plan.

Probably not he was 13 and she was 11 when he was banished. its certainly possible but Mai was hardly the age for it. And Zuko was dealing with the grief from the loss of his mother, and new found duty as heir of the throne.

Again I guess it's possible but I find it unlikely.

Context shows that Mai was crushing on him since they were little, but they probably didn't get together until after Ba Sing Se.

No they weren't. In fact as shown in a flashback he barely noticed her. Him saving her from the apple was just him being chivalrous. He never mentioned her at all during the show until season 3. Besides as someone else said he was 13 so shed have bren 11 or 12.
