Debt consolidation companies usually just charge fees for stuff you can do yourself. Have you tried calling your other cards directly for hardship programs before going that route?

When your unemployed is the best time to file bankruptcy. If unemployed longer than 5 months, its definitely something to consider.
You are likely just going to go deeper in debt if you are only getting 1k/month unemployment.
Ch7 uses most recent 6 months for eligibility calculations.
There are other options at this point, just putting it out there to keep in mind.

Keep calling Chase back, you might get a different rep who's more helpful. Timing with that payment probably screwed you but it's worth trying again
