Controversy Changes to the Subreddit heading into 2022

Hi r/Gastroenterology members!

As some of you have noticed (and posted about) there has been a slide in post quality recently with a lot of breaking of rule 1. Mod team is on the case! Here are some things to expect in the near future:

  1. Tighter moderating of posts
  2. Introduction of post flairs (please use these, will not be enforced at least initially) to help delineate posts subtypes at a glance.
  3. Will have recurring weekly posts on different subjects such as latest interesting publications which can be discussed or further added to
  4. Please message mod team, or me directly, if you have any other ideas that you think may make this a more interesting community and we will work to make those changes!

Aside from blatantly breaking the sub's rule #1 that this isn't a place for your convenient internet medical advice but rather to discuss the specialty of gastro, I don't want to scroll my front page over breakfast and come across a literal picture of feces.

GI exam involving breasts?

This is going to sound odd, but I went for a quick follow up to my endoscopy/colonoscopy today. Overall the findings were fine and he did a quick exam of my abdomen but then asked to lift my bra on one side and took a look at my right breast. He didn’t touch it, maybe he pressed on the area beneath where the band/underwire sits, I can’t clearly remember bc I was trying to ask him about a random pain below my rib cage. He didn’t offer an explanation and it didn’t occur to me that it was odd until later tonight. Is there any reason this would be part of a quick exam?

Question about dysbiosis and IBS

It is now becoming common for gastros to say they think IBS is caused by some underlying dysbiosis. Up to 70% of cases are due to dysbiosis apparently

Dysbiosis has been linked in several research papers to CrC and IBD. So why is IBS seen as harmless if over 70% of cases are assumed to be a result of dysbiosis?

As mentioned above, dysbiosis is not harmless and has causative links to IBD and CrC. So maybe IBS isn't as benign as they say and does indeed lead to these other conditions?

Chronic burping

Before you come at me about seeing a doctor over this issue, I have. I'm not a millionaire that can afford doctors.

Maybe a decade ago I started dealing with chronic burping. In the morning, I'll feel an intense pain in my upper abdomen that feels like an extreme hunger that's followed by a succession of burps. As if they came out of no where. I'll be burping here and there basically all day, sometimes every few minutes. I'm not drinking carbonated drinks often either. Not every day. I'm wondering if it's a gallbladder issue because I also deal with chronic nausea and diarrhea. Has anyone experienced anything similar and got answers? I know I'll need something more invasive done like a colonoscopy and endoscopy, but at this point I don't have the money to consider it. I'm so sick of burping.

Is there a link here? Gastro issues since Feb and blood clot on renal vein. Please help my doctors are writing me off with stress related IBS.

40F UK. Feb I had a change of bowel habits mushy stools orange, yellow sometimes green ,constipation and pain in lower left abdomen and sloshing noises in lower left abdomen bloating constantly all still on going and lost some weight unintentionally sometimes pain under left rib and silent reflux. Test I have had done- X2 CT scan with contrast of the chest, abdomen and pelvis found blood clot on renal vein. MRI with contrast of pelvis found fibroids in uterus. Ultrasound of pelvis found fibroids. Colonoscopy found polyps came back benign. CRP blood test normal. CA125 15 Kidney, liver, full blood count B12, A1C all normal. Calprotectin stool sample 30 H pylori negative from stool sample Celiac negative from blood test No blood detected in fit test and never seen in stools. Colorectal team have now referred me back to GP and said I have stress related IBS but obviously this doesn’t cause weight loss and blood clots. Please help because now I am really stressed and worried about what is going on inside my body. Thank you

Brauche Rat um meine kranken Oma (Gastroenterologie)

Meine Oma 78 Jahre alt hat Leberhepatitis und das hatte zugolge das sie wasseransamlungen in Beinen und Bauchtanz bekommen hat ganz schlimm ist das im Bauch da der Bauch langsam steinhart geworden ist und die Ärzte hier in Rumänien sagen sie können die Flüssigkeit nicht rauslassen weill das sonst schlechte Auswirkungen auf das Gehirn haben kann ich vertraue nicht ganz drauf was die mir da gesagt haben deswegen wollte ich mir hier mal ein Rat holen ihr Zustand ist sehr schlimm das Hepatitis hat sie seit 15 Jahren unbehandelt gehabt und mittlerweile ist sie vom Kopf bis zur Brust komplett gelb angelaufen die Augen sind dunkelgelb ich weis nicht mehr weiter was ich machen soll die haben mir eine Riesen Liste an Medikamenten gegeben die sie über den Tag bekommen muss was aber nur mit der Leber zu tun haben zu der Flüssigkeit im Magen haben die gesagt das man nichts machen kann was ich aber im Jahre 2025 tatsächlich nicht glauben kann freue mich über jede Idee oder hilfreichen Kommentar

Biofeedback therapy for adult intussusception & other ODS - how exactly does it treat dyssynergia?

Hi there, I’ve met a couple of patients diagnosed with functional rectoanal intussusception that are referred for biofeedback as a first line treatment instead of STARR, Delorme etc.

I understand research backs this for long-term outcomes (to be clear, I’m not questioning the appropriateness of these referrals!) it’s just I’m a bit confused how biofeedback actually works for patients with intussusception? Surely the colorectal telescoping is entirely involuntary.. so how can it be ‘trained’ away?

Hello dear health experts,

My father has been struggling with a long-term health problem. He often experiences stomach pain, stomach cramps, and gas issues. In our local language, we call this condition “naara ukhad jana”, which means the stomach makes sounds like gud-gud-gud (gurgling noises). Because of this, he feels extreme laziness, body pains, and loss of energy — he doesn’t feel like doing any work.

He has already consulted almost every available doctor in our area and taken dozens of medicines, but nothing has cured him. Sometimes the strong medicines even created new problems instead of solving the main one.

History & lifestyle:

He used to do heavy physical work, lifting weights like 20–50 kg or even more regularly.

Currently, he runs a grocery store, where he still has to lift heavy bags (like 30–50 kg rice sacks), even though he tries to avoid it.

Whenever he lifts something heavy, the stomach cramps and gas issues worsen, leading to vomiting, extreme laziness, loss of appetite, and body pain.

This may sound like a small issue, but for him and for us, it has become a very big problem. I can’t bear to see him suffering and still trying to work.

I know some of you may recommend complete rest and avoiding heavy work — but unfortunately, he cannot stop working because of family responsibilities. I am not yet capable of taking over his role, so for now, I need a solution to at least manage and reduce his condition before it gets worse.

One more thing — we have a traditional practice here called “naara ukhadna” (a type of stomach massage). It gives temporary relief when the pain becomes unbearable, but it is not recommended for daily use by either doctors or elders, as it may cause new problems if done too often.

👉 Please, if anyone knows of a permanent or long-term solution, treatment, or management method for this problem, kindly guide me. Any advice would mean a lot to me and my father.

Constant bloating

Hi, i don’t know if this is the right sub. I (24F) have been struggling with constant bloating for the last weeks and I also had the same thing happen to me in 2022. The bloating lasts for months, and I wake up in the morning bloated and go to sleep still bloated. I noticed it happening for the first time whilst assuming anti reflux medication (prescribed), since I struggle with heartburn. Rn I’ve stopped taking them cause I finished the cycle.

My weight is normal (50kg-160cm), I wouldn’t say that my life style is sedentary but I def could move more, and I try to eat as best as I can. I don’t drink much alcohol. Sometimes I smoke.

I do not understand why I’m constantly bloated. I don’t have (or at least i don’t think so) IBS, since I never have diarrhea or nausea, I can eat anything I want but I’m constantly bloated. Also, as far as I know (exams from 2022) I’m not lactose intolerant nor gluten sensitive. What could be the cause of this? What could I try?

feeling confused

disclaimer: i am not asking for a diagnosis just wondering if anyone else with ibs has these symptoms?

hey guys! So I recently visited a GI for a referral of alternating diarrhea and constipation. those are my most minimal symptoms and listed below are my most consistent on/off symptoms - dark diarrhea with black and white flakes - constipation alternating with diarrhea - pencil thin stool - White flakes in stool - blood after excessive bowel movements - struggle with bowel control - severe abdominal bloating - pain near belly button and both sides of abdomen - nausea/vomiting - very dark stool - fatigue - elevated bilirubin? -low grade fever -pain in joints -excessive mucus in stool

after speaking to the doctor for like 2 minutes he said its just ibs. i informed him that direct family members have been diagnosed with various ibd and he scheduled a colonoscopy a month from now along with other tests. im just worried that im not moving in the right direction

Colon

I got a colonoscopy last year at the age of 21 and it came back clear. I am now 23 and I’m experiencing some new symptoms of loss of appetite, feeling full very quickly after small meals, feels like someone is twisting my belly from the inside, bloating, burping and pain on the left side of my belly and pelvic area. My insurance won’t allow me to get another colonoscopy because of my age and recent clear one last year but my anxiety won’t let me rest with these new symptoms. I want to add that I am a heavy weed smoke and ChatGPT I’d suggest CHS. My anxiety got the best of me and I went to the ER due to some burning in my tummy. I got a CT scan and they found nothing but ovarian cyst I already knew about but obgyn doctors said it was small it probably wouldn’t cause much pain. I am scared that I may have colon cancer. I went to my primary care doctor and she suggested my symptoms may be irritable bowel syndrome and gerd. Got a barium swallow and came back positive for gerd and now I am on a PPI. What’s your advice do you think it’s just gerd or signs of colon cancer? I’ve had a grandmother with colon cancer in her 40s so that’s adding to my anxiety. Thanks

Chronic Gastritis - NHS Gas Lighting - IMO (Intestinal Methanogen Overgrowth)

UK NHS have been Gas Lighting that SIBO and IMO is all in my head . I have tested positive for IMO with levels up to >80 ppm Methane within 15 to 30 mins of breathtest. My baseline was 55 ppm which both results showing chronic methanogen infestation. This is following years of Hydrogen SIBO.

My GP referred me to Gastro Team in my local hospital who perfomed an endoscopy that revealed chronic gastritis. They perfomed biopsy that revealed no H. Pylori, completely ignored my IMO condition and discharged me back to my GP with the advice to reduce my alcohol intake. This is despite me telling the Dr performing the endoscopy that i gave up drinking 10 years ago.

I made an offical complaint through PALS, and have since received an opology from Gastro Team that i should not have been told to reduce my alcohol intake, given that i gave up alcohol long ago. They also apologised for discharging me back to my GP who is powerless to treat SIBO / IMO as Rifaximin is cost prohibitive to prescribe in UK on the NHS. They have now referred me to a senior Gastroenteroloist and given me an extended appointment on 1st Sep to fully explore my SIBO / IMO condition.

This just goes to show that you do not have to just accept this gas lighting from GP's and Gastroenterologists, and should push for support with diagnosis and treastment of SIBO and IMO.

One thing that remains a mysetry to me is that if you search the internet, there doesn't appear to be a direct link between SIBO / IMO and Gastritis. I've asked Dr Mark Pimentel if he has any research data / papers on this.

In the UK, we're not asking for miracles, we are asking for:

  1. Formal clincial recognition of SIBO and IMO as chronic, treatable conditions
  2. Accessible diagnostic testing through NHS
  3. Affordable treatment access, including NHS pricing reform and prescription coverage for drugs like Rifaximin, and Neomycin
  4. Consistent advice on Herbal Microbial, Pre-biotics, Pro-biotics, and diet treatments as the internet including Reddit is full of contradictions on what to do
  5. National clinical guidance to prevent the years-long cycle of dismissal, misdiagnosis, and fragmentation
  6. Mitigate the two-tier healthcare system emerging for those that can afford private healthcare treatment for SIBO / IMO, and those condemned to years of chronic suffering simplybecause they cannot afford treatment.

The above does not just apply to the UK. This is a global healthcare problem that is being ignored when 60% of IBS is considered to be due to SIBO or IMO.

HELP! Loose Stools for 7 days now? Don’t know what to do!

I am writing this on behalf of my mother, she got severe diarrhoea about a week ago because of which we had to admit her in the hospital under the care of a Gastroenterologist, The main issue that she was facing was :- 1. High fever when admitted around 102F 2. Loose stools - green in colour 3. Abdominal pain (not much)

While she was in the hospital she was first given Monocef 1g (Ceftriaxone Injection) on the first day.

On the second day since there was no improvement and the stools were still not stopping she was given Meropenem Injection, and given two tablets of loperamide.

This was continued and the stools stopped for some time. She was discharged from the hospital, but still she is having loose stools, She is still on faropenem tablets.

We are totally lost, we don’t know what to do now..

Do anyone have any idea what could be the root cause for this her blood work and ultrasound is normal!

Granuloma pathognomonic for Crohn’s disease?

Endoscopy in Dec 2023 revealed severe inflammation with small non-necrotising granuloma and mild esophagitis. Was placed on 40mg Omeprazole SID. Recheck endoscopy in Sept 2024 revealed mild esophagitis, chronic gastritis with no granulomas seen. Colonoscopy showed mild, chronic colonic inflammation.

Current diagnosis: IBS. However, Merck Manual tells me granulomas on gastric biopsy are pathogenicmonic for Crohn’s disease, but not always found. My gastroenterologist says since there were no granulomas on repeat biopsy, I don’t have Crohn’s disease. Is this correct?

Currently taking 20mg omeprazole BID + fiber daily, symptoms have not improved since original presentation in 2023 despite improved endoscopy findings. Symptoms include GERD with burning sensation after swallowing liquids, near constant bloating and belching, frequent BMs/ diarrhea which worsens with trigger foods, and if I miss a day of omeprazole, my mouth fills with ulcers (especially under the tongue and tonsils). Can count the number of times I’ve burped in my life on 2 hands until 2023, now I exceed that hourly. TIA!
