Question respecting second yellow

Hi,

  1. If someone gets a yellow when he is still sitting on the bench before getting subbed on, and gets another yellow when he has been subbed on, does that equal a red card?
  2. What if he is a player-manager and the first yellow was received in a managerial capacity?

Does anyone have any example of a player getting a yellow and then later a straight red without the second yellow? I know its possible but can't think of any examples.

Yes it happens all the time, it happened this weekend. I can't remember who it happened to but it was a fairly prominent game, there was a post in arrr soccer where the title was wrong (it said second yellow when it was actually a straight red)
