The fire nations royal family have no other names, only their first. They don't need them, and in situations where they would need to specify which Iroh they're talking about, they can use their title. In Irohs case it's still prince, since until zuko or azula have kids he's technically third in line for the throne.

Not a lot of people in ATLA seem to have last names, Toph is pretty much the exception. Everyone else seems to be recognized by their titles/accomplishments, or by their nationality/tribe.

His name is just Prince Iroh. He'd be recognized by General Iroh, or Dragon of the West as well.
