Heres the clutch, what does it tell yall. Maybe 10-15k miles on it

This looks like something was in there with it, FOD damage.

How's the TO bearing and input shaft? Any slop on the input shaft?

That's my thought as well. There was definitely something in that clutch. Had the same thing happen on my car and it looked pretty similar.

When I replaced the missing TO bearing 5 years ago they gave me a Bushing. I didnt look at it when I replaced my ballistified clutch becasue I was in a hurry to get the car back together and it drove fine for about a year then 6 months ago it started having issues shifting at high rpms, when that went away I started getting a really bad vibration in some gears.

The input shaft is noticeable grooved and the bushing is bored out to about 3/4inch instead of the .5inch spec.

Im having the transmission opened by a shop for a full inspection and to replace anything that might have been hurt by the vibrations and check the shifter linkages becasue ive shifted hard on it through some of the binding. No metal shavings were found in the fluid though 3 months ago when I tried flushing it to help it shift better.

TO bearing or pilot bearing? You cant replace a throw-out bearing with a bushing. You can replace a pilot bearing with a bushing which is a smart idea. (Throw-out bearing is what operates the clutch, the pilot bearing is what sits in the back of the crank that supports the input shaft)

My bad. The bearing on the slave cylinder still feels smooth and doesnt look worn. The bushing i was given to replace the pilot for the input shaft is wallerd out and the input shaft had scarring on the nose. Also the input shaft has about a 16th a inch wobble up and down, not sure if that amount of play was there before this happened.

If the pilot bearing/bushing was missing last time you did the clutch, 95%+ the input shaft was already fubar'd; especially with what you've been describing.

Honestly you bought a car that had been ridden hard and put away wet after lots of abuse. Trans was fubard which is probably why they sent it down the road.

Probably. I got it at 196k miles and sitting at 240k miles on it now. A little bit of valve train rattle and the clutch problems is all ive had out of it so far.

Lol. This one bugged me because its only been 2 years and im dropping the transmission because its vibrating horribly and cant shift at high rpms.
