Putting those pieces together he likely has congestive heart failure that's also causing kidney failure. Blood thinners cause big bruises like that, and the catheter may be to help drain urine to alleviate pressure on his kidneys. Or he's on diuretics that aren't working very well and it's making him urinate a lot and he's having trouble holding it. Pair that with his obvious mental decline and the guy is in terrible shape.
