The "loli" word for pedophilic Japanese art (from the word lolita) doesn't have the double L, and I've never seen/heard it used offline, so I never make that association.
Lollipop is still pretty common in English for the candy (although in my experience "sucker" has become more common, but I feel like that's a slightly different candy- suckers are round and lollipops are flat, or am I just crazy for thinking that?).

Yeah the "secret 45" thing seems like jumping at shadows. If that's what they wanted, they probably would just put his face on it (why be subtle when you've committed already to a swatstika).
Yup, that's it - I think you can make out the bulgarian letters Đ and Đ now, which makes the changes to the classic swastika make sense (assuming this is really a bespoke flag, made with himself in mind)

This looks like a protest flag of some kind against the last Trump administration.
Looks like a 4 and 5 rammed together in order to make a swastika. Trump was the 45th President of the United States and made controversy at the time when he told the Proud Boys, a known white nationalist group, to "Stand back and stand by".
It looks like a combo of 45 (trump's first term) and the swastika. Otherwise from the photo here's no way to tell if a supporter or a critic put this up.
Edit: OP said they saw it on an alt-right looking profile. So, assuming it's not a satire account, it looks like it's a supporter. More than that, an open and out nazi. Pretty gross.

But to answer the question this looks similar to a 2017 protest symbol design by Mike Mitchell. Albeit without the no symbol over the top and an extended arm for a 7 or cross as others have suggested.
I don't know for sure, I'm assuming this is swastika + upside down cross.
Might be from some black metal bend, there's quite a few of them that are neo nazies and proud of it, some band members even founded cults that worship Hitler as a diety.
